The invention disclosure form is an important first step in the process of ultimately protecting and licensing your invention. With the information you provide, we can evaluate your invention's commercial potential and our ability to protect the invention through patents or copyrights.

Instructions:
- Your disclosure form must be completely filled out and signed by all the appropriate parties.
- Please submit your form by campus mail, facsimile or email -- If you email this form, you must follow up by sending or faxing a signed, physical copy of at least the second page. We need to keep both copies for our records.
